Cervical Mucus Method: Effectiveness, Pearl Index, and Use

Updated on: Author:

A brief summary of the cervical mucus method (Billings)

Pearl-Index
5–35  (depending on the application)
Suitable for
Experienced and highly diligent users who monitor and document their progress daily; with a good understanding of data analysis.
Less suitable for
irregular cycles, breastfeeding/shortly after childbirth, perimenopause, acute infections/medications, frequent changes in hygiene products.
Common mistakes that compromise security
  • Irregular observation or documentation (days are skipped).
  • Mistaking cervical mucus for vaginal discharge, semen, lubricant, or residual menstrual blood.
  • Observations made at different times of day; no comparison under identical conditions.
  • Ignoring confounding factors (infections, medications, vaginal dryness).
  • Incorrect assessment of the "peak" or premature identification of infertile days.

Note: Less reliable when used as the sole method of contraception. The symptothermal method (Temperature + (cervical mucus) increases reliability.

The cervical mucus method alone is considered unreliable for natural contraception. Most women find it difficult to correctly identify cervical mucus. Accurately assessing cervical mucus requires a great deal of experience and discipline. The Pearl Index for this method is only 5–35, meaning that, for example, 5 or 35 out of 100 women will become pregnant despite using this method!

Greater reliability when used in combination—the symptothermal method

For safe, natural contraception, we recommend the symptothermal method. This method combines the temperature method with the cervical mucus method. In the OvulaRing app, you can accurately track changes in your cervical mucus, giving you a clear picture of exactly where you are in your cycle.

The “cervical mucus” method, also known as the Billings method, was developed by the Billings, a married couple of doctors. This method determines your fertile days solely based on changes in your cervical mucus throughout your cycle. You observe your cervical mucus during the day and note down in a cycle chart in the evening what the mucus was like that day. This involves using your senses—touch, feel, and sight. Just as every woman has a unique cycle pattern, it seems that cervical mucus patterns also vary from woman to woman.

Throughout your cycle, hormone levels in your body fluctuate, which also causes your cervical mucus to change. Nature has cleverly designed this system: the purpose of cervical mucus is to prevent pathogens from entering the uterus on infertile days and to provide an optimal environment for sperm on fertile days.

How does cervical mucus change throughout the menstrual cycle?

After your period, there is initially no discharge, and the vagina feels dry. In the days that follow, cervical mucus is noticeable only as a “moist sensation,” but it is often not visible or detectable at the vaginal opening. Shortly thereafter, it becomes sticky, stringy, thick, and clumpy. At this point, you are not fertile. The closer you get to ovulation, the more estrogen is produced, and the amount and consistency of the cervical mucus change progressively. The discharge changes as follows over the course of the cycle:

If you want to use natural family planning (NFP), you must either abstain from sex during your fertile window or use barrier methods such as condoms or a diaphragm. The last day with the best cervical mucus quality for you is referred to as the peak of the cervical mucus sign and marks the most fertile phase. The end of the fertile phase is marked by a change in cervical mucus quality and a rise in basal body temperature.

Safety of the cervical mucus method

The Pearl Index for the cervical mucus method is 5–35. This means that 5–35 women become unintentionally pregnant over the course of a year while using the cervical mucus method. The cervical mucus method is therefore considered one of the less reliable methods of natural contraception and is not recommended as a method on its own. Most women find it particularly difficult to interpret cervical mucus correctly, and the method requires a great deal of experience and discipline.

For safe, natural birth control, we recommend that you practice Symptothermal Method, which combines the temperature method with the cervical mucus method. In the OvulaRing app, you can accurately track changes in your cervical mucus, giving you a clear picture of where you are in your cycle.

FAQ: Frequently Asked Questions About Cervical Mucus and the Cervical Mucus Method

Cervical mucus is a secretion from the cervix. Its consistency and amount change throughout the menstrual cycle and can indicate fertile or infertile days.

When used on its own, it is less reliable; the Pearl Index is approximately 5–35 and depends heavily on the specific application (experience, consistent observation).

From “dry/barely noticeable” to “creamy/whitish” to “clear, stretchy, and stringy” just before ovulation; after that, it becomes thicker and creamier again.

During the most fertile phase, the cervical mucus usually clear, thin, and can be spun between the fingers. The “peak” indicates the highest fertility.

Increasingly moist to watery and then ready/spinnable. Before that, it may be creamy/whitish or thick.

Yes, stretchy, transparent slime is a powerful Fertility signs. It typically occurs shortly before ovulation.

Cervical mucus changes cyclically and, among other things, after Feel, appearance, elasticity assessed. Vaginal discharge can also have other causes (e.g., infections). If you experience any symptoms, always consult a doctor.
