The cervical mucus method alone is considered unreliable for natural contraception. Most women find it difficult to correctly identify cervical mucus. Accurately assessing cervical mucus requires a great deal of experience and discipline. The Pearl Index for this method is only 5–35, meaning that, for example, 5 or 35 out of 100 women will become pregnant despite using this method!

The “cervical mucus” method, also known as the Billings method, was developed by the Billings, a married couple of doctors. This method determines your fertile days solely based on changes in your cervical mucus throughout your cycle. You observe your cervical mucus during the day and note down in a cycle chart in the evening what the mucus was like that day. This involves using your senses—touch, feel, and sight. Just as every woman has a unique cycle pattern, it seems that cervical mucus patterns also vary from woman to woman.
Throughout your cycle, hormone levels in your body fluctuate, which also causes your cervical mucus to change. Nature has cleverly designed this system: the purpose of cervical mucus is to prevent pathogens from entering the uterus on infertile days and to provide an optimal environment for sperm on fertile days.
After your period, there is initially no discharge, and the vagina feels dry. In the days that follow, cervical mucus is noticeable only as a “moist sensation,” but it is often not visible or detectable at the vaginal opening. Shortly thereafter, it becomes sticky, stringy, thick, and clumpy. At this point, you are not fertile. The closer you get to ovulation, the more estrogen is produced, and the amount and consistency of the cervical mucus change progressively. If you want to use natural family planning (NFP), you must either abstain from sex during your fertile window or use barrier methods such as condoms or a diaphragm. The last day with the best cervical mucus quality for you is referred to as the peak of the cervical mucus sign and marks the most fertile phase. The end of the fertile phase is marked by a change in cervical mucus quality and a rise in basal body temperature.
